Member ID Card
Good things come in small packages. You’ll receive your member ID card—a pocket-sized description of your health plan—in the mail after you have enrolled. You will want to carry it with you, because you will use your member ID card every time you receive care.

You don’t have your ID card in your wallet? We’ve got you covered. You can access a digital version of your card on the Blueprint Portal member portal or our Blueprint Portal app if you’re already in the doctor’s office.

Member ID number
Helps us identify who you are, the type of plan you have and how your claims are paid.
Deductible (INN DED/OON DED)
The amount you pay for services before your benefit plan begins to pay for covered expenses.
Non-essential Health Benefits Deductible (INN NON EHB DED/OON NON EHB DED)
How much you pay for certain services that aren't considered essential before insurance pays.
Group number
Used to check your plan’s benefits
Rx Categories (RX BIN, RX PCN, RX GRP)
Used by pharmacies to determine what's covered, apply your discount and file your claims.
Copay
A fixed amount you pay for covered health care service, usually when you receive the service. The amount can vary by the type of covered health care service.
RX Copay
A set amount you pay for your prescription.
Maximum Out-of-Pocket (INN Max OOP/OON Max OOP):
The most you could pay during a coverage period (usually one year) for your share of the costs of covered services.
PPO or TRAD label
Represents your access to the national BlueCard®️ network that enables you to find and receive certain levels of healthcare benefits when you live or travel across the country. The label PPO indicates you have access to our Preferred Provider Organization (PPO) throughout the country, while TRAD indicates you can access Traditional BlueCard network providers but not the PPO network outside your geographic service area.
